Vaccination registers include the child's name, sex, birth date and place (or age according to the Vaccinator's Certificate), name and surname of the father or, (if the child is illegitimate) of the mother, rank, occupation, or profession of such parent, when registered, when given, to whom given, date of medical certificate of successful vaccination, name of medical person by whom the certificate is signed.
